People who earn over certain thresholds pay a penalty for not having private hospital cover. The Medicare Levy Surcharge (MLS) applies to singles earning over $90,000 and families with a combined income over $180,000 without private hospital cover.

The levy payable is a percentage of their taxable income – starting at 1% (or around $900 a year) increasing to as high as 1.5% for higher income earners.
