What is the Care Coordination Program?

Our Care Coordinator can assist you to navigate and coordinate your care needs to better manage chronic disease and help keep you at home.

As a member-owned health fund our focus goes well beyond simply providing health insurance. Our aim is to assist our members identify their health and wellbeing needs and help coordinate access to services that will keep them happier and healthier in the long term.

Our Care Coordination Program can provide additional support for TUH members who live with a chronic illness, or have recently been discharged from hospital.

Our Care Coordinator will identify that you have had a recent hospital stay and may benefit from the program.

We can also help coordinate other healthcare support services to assist with your recovery such as medical aids, pharmaceutical deliveries and much more. The Care Coordination service is complimentary to eligible members and participation is voluntary.

How the program works

Depending on your circumstances, a TUH Care Coordinator may contact you prior to planned hospital admission or after you are discharged from an unplanned stay. They will undertake a comprehensive phone assessment and develop a care plan to identify additional support and services that will assist in both your recovery and long term wellness. Any in-home treatment must be agreed to by your medical team first.

Our goal is to ensure you can return to the comfort of your own home as soon as possible while still receiving the best care available.

What services are available?

Post hospital discharge follow-up program

Getting out of hospital is great but staying out of hospital is even better. Often hospital re-admissions occur due to a lack of support at home. An inability to attend follow up appointments, fill scripts or other unforeseen circumstances can impact your ability to achieve a complete recovery and may result in a return to hospital. 

Our Care Coordinator can assist with navigating support and service options after a hospital stay.

Hospital care at home

We understand hospital stays are disruptive so whenever possible, under the guidance of your medical team, you can receive hospital care at home including wound and medication management and selected home nursing interventions.

Rehabilitation at home

Reducing your hospital stay by accessing in-home rehabilitation services can provide priceless support during your recovery. Eligible members who have approval from their treating doctors can receive rehabilitation services from health care professionals in the comfort of their own home.

Who can use this service?

Members are eligible for care coordination services if they have hospital cover and have served all waiting periods, are over 18 years old and have an identified chronic condition.
