TUH has contracted Home Support Services Pty Ltd (HSS) to provide our health management programs (eg. Health-e-Profile, Home Care, SupportLine and Disease Management). Home Support Services (HSS) is committed to complying with all applicable privacy laws including the Privacy Act 1988 and the National Privacy Principles. HSS respects the privacy rights of members, customers and stakeholders and is committed to ensuring any personal information (including sensitive or health information) that you provide when participating in a Strive for Health service is protected and remains confidential, HSS is also bound to comply with all applicable privacy laws including the Privacy Act 1988 and the National Privacy Principles. As such, we have strict policies about who can see and use your information.
How we use your information:
Your personal and health information will only be collected, used and disclosed to enable HSS to adequately provide you with the services related to health assessment and health management (Services) and in a manner consistent with applicable privacy laws.
HSS utilises the services of Medikeeper Inc (Medikeeper) to develop and maintain the Strive for Health website. All personal and health information collected by HSS via the Strive for Health website is stored by Medikeeper on its servers based in the United States. Medikeeper is bound to comply with HSS' directions in relation to the security of information and takes all possible precautions to safeguard their data bases and information.
HSS ensures that access to personal and health information is limited to authorised personnel for the purpose of the provision of the intended services and that all authorised personnel comply with applicable privacy laws.
All reasonable steps are undertaken to protect the personal and health information HSS holds from misuse and loss and from unauthorised access, modification and disclosure.
De-identified data may be used for statistical or research purposes by HSS or your Private Health Fund.
We only may disclose your personal or health information about you to third parties for the purpose of managing, delivering and administering the provision of these services.
If you do not provide us with all information that we request about you, this may affect our ability to provide the services we may be unable to provide the services and information as planned.
There are some situations where we may disclose your personal or health information, without your consent. For example, in an emergency situation, we may be required to release medical information about you to aid emergency treatment. Also in certain circumstances, HSS may be required or permitted by law to release information about you. If any of these circumstances apply we will advise you as soon as practicable after the information is released.
You have the right to correct your information if you believe it to be inaccurate or out of date or as otherwise permitted by applicable privacy laws.
