Health e-Profile
What is your Health e-Profile? It’s a simple survey you complete about your health care needs, that allows us to unlock targeted value and rewards that you’ll actually find useful.
We know you're busy, so it's short and simple!
As a bonus, your Health e-Profile will also help identify any risk factors in your life that could contribute to potential illness or health conditions later.
Fill out your Health e-Profile once a year, and if you’re eligible, we’ll give you an Active Health Bonus for you to put towards your choice of extras. All for just 10 minutes of your time – sweet deal, right?
And if we do find any risk factors, you may receive health information to assist you in reducing your risk or if you receive a moderate to high risk score one of our Health Care Services team could get in touch to talk you through resources and programs that could help you to shore up your health and reduce your risk.
Don’t worry about remembering to fill in your Health e-Profile – we’ll remind you once a year when your survey is due, so you never have to miss out on your bonus.
