Home care programs
TUH has contracts with a number of providers of home care programs that provide tailored treatment options to keep you out of hospital or help you make a speedy recovery at home after illness or an operation.
How does it work?
Before or during your hospital stay tell your doctor and nursing staff that you can access TUH’s home care programs. They’ll assess your situation and advise if the home care program is medically appropriate for you. With your consent, your doctor or nursing staff will contact a program coordinator (registered nurse), who works closely with you and the hospital to organise the appropriate services.
Examples of available services:
• Antibiotic therapy
• Palliative care
• Negative pressure therapy
• Acute mental health
• Orthopaedic rehabilitation program
• Respiratory rehabilitation program
• Cardiac rehabilitation program
• Reconditioning rehabilitation program
If you have hospital cover you can participate in a home care program so long as your cover includes benefits for that treatment or procedure.
A waiting period of two months applies.
TUH’s home care programs are available throughout our participating hospital network. TUH is not involved in any clinical assessment.

The program is designed to complement services provided by your existing health providers. A health coach will call you, discuss your needs and recommend a suitable program. They will keep in contact with your treating doctor to monitor your progress.
